How to register as a patient
Your Named Accountable GP
As from 1st April 2015 NHS England requires all practices to allocate a ‘named GP’ to all patients who is responsible for your overall care at the practice and will take lead responsibility for the coordination of all services. You will still however be able to see any GP within the practice.
Please contact the practice if you wish to know who your named GP is, and if you have a preference as to which GP that is, the practice will make reasonable efforts to accommodate this request.
For more information on how your data is used and shared within the NHS please click here for the GP fair processing Notice, and to download opt out forms please see the link at the side of this page
