Integrated Health Team
The community nursing team at Oakley & Overton Partnership now works as part of an ‘Integrated Care Team’ alongside GPs, social workers, community mental health nurses and other therapists. Integrated Care Teams (ICTs) have been set up in many areas of the country and consist of different health professionals working closely together to provide effective and co-ordinated care to mainly home-based patients with complex needs. Locally six integrated care teams have been established jointly by the North Hampshire Clinical Commissioning Group and Hampshire County Council, Adult Services working closely with the Southern Health NHS Foundation Trust. Overton and Oakley surgeries are covered by the Rural West team who are based at Tadley. They also cover Kingsclere and Tadley Practices. The team is led by Sheila Gascoigne/CPN, Dale Macdougall/Community Matron and the lead GP is Dr Roisin Ward. Dale Macdougall is also supported by other members of the ICT team who provide skilled nursing care to housebound patients with an identified nursing need (above 18 years of age), such as those recovering from the effects of illness or surgery, and those with long-term or terminal illness. Members of the team currently are: Sister Debbie Kerswell, Staff Nurses: Kim Major, Bhia Mukandasi, Heather Tyas, Eva di Romana and Health Care Assistants: Sara Lee, Debbie Burridge, Jane Seaton and Amanda White. The Administrator for the team is Ingrid Botha. Our Integrated Care Team meets regularly with the practice to discuss patients with complex needs so that the care offered is well coordinated and appropriate to the patient’s needs. We aim to support patients in their own homes when possible and so prevent unnecessary or prolonged hospital admissions. |
